East Tremont: We want you to vote on the top health priorities in your neighborhood. Join us on Tuesday, Jan. 5 at 6 p.m. to have your voice heard.

In New York City, health outcomes differ dramatically between communities — poor health outcomes cluster in places where resources and opportunities are limited.

No one’s health should be determined by his or her zip code, race, or income. This is unjust and avoidable.

What is Take Care New York 2020?

Take Care New York 2020 (TCNY 2020, PDF) is the City’s blueprint for giving everyone the chance to live a healthier life. Its goal is twofold — to improve every community’s health, and to make greater strides in groups with the worst health outcomes, so that our city becomes a more equitable place for everyone. Unlike previous Take Care New York plans, TCNY 2020 looks at traditional health factors as well as social factors, like how many people in a community graduated from high school or go to jail.

How can I support Take Care New York 2020?

Join us at Community Consultations this fall and winter, in dozens of neighborhoods across the five boroughs, as Health Department leaders meet with communities to talk about TCNY 2020 and to hear from residents directly on their neighborhood’s top health priorities. The input we receive from community members will be vital for the next phase of TCNY 2020, which includes community-based action planning.
